Schedule M15 - Estimated Tax Underpayment FormIf you failed to pay or underpaid your estimated income tax for the previous tax year, you must file Schedule M15 to calculate and pay any interest or penalties due to the Minnesota Department of Revenue.

Form M14 Instructions - Estimated Income Tax InstructionsUse this instructional booklet to help you fill out and file your 2012 M14 estimated income tax vouchers. Don't forget the due dates are: April 15 (first quarter), June 15 (second), Spetember 15 (third), and January 15 (fourth). Note: If you file your 2012 M1 by January 31, 2013, and pay the entire balance due, you do not have to make the fourth installment.
