Schedules A and B are addendums to the Iowa 1040 for reporting specific deductions (such as taxes, medical expenses, donations, etc) and reporting dividend and interest income. If you wish to claim any of these deductions or have earned any qualifying interest or dividend income, you must file these schedules with your 1040.

File form IA 8453-IND to declare your intention to submit an E-File Tax Return 41-011. This form includes consent for electronic payments and direct-deposit of Iowa income tax refunds.
02/2013Form IA 1040V - Payment Voucher
If you submit your tax payment as a check or money order, enclose a completed Form IA-1040V with your payment. Iowa encourages all taxpayers to submit their payments electronically for faster processing.
